# *Rheumatology Charge Capture & Medical Coding: Stop Missing Revenue*

*A patient can get her hands back. Your practice should not lose the revenue that helps make that care possible.*

Picture a 54-year-old patient who has not opened a jar comfortably in years. Rheumatoid arthritis turned ordinary moments—buttoning a shirt, making breakfast, tending a garden—into daily challenges. After years of symptoms, consultations, and treatment decisions, she finally reaches a rheumatologist. Months later, a biologic therapy is helping. She's gardening again.

That is why rheumatology matters. But behind every successful treatment is another demanding workflow: documentation, biologic infusion billing, insurance authorization, medical coding, charge review, and reimbursement.

For rheumatology practices, **charge capture is more than entering a procedure code**. It's the process of translating the care actually delivered into complete, supportable, bill-ready charges. When those details live in disconnected systems—or are reconstructed days later—legitimate revenue can be missed, delayed, or denied.

## Where rheumatology practices lose revenue

### 1. Biologic infusion doses and billing units don't match

Many infusion therapies require careful documentation of the prescribed dose, actual dose administered, drug supplied, and applicable HCPCS billing units. For weight-based therapies, the patient weight used in dose calculation may be essential to the record. Drug acquisition, administration, and billing also need to line up.

If the infusion record and charge entry don't tell the same story, the coding team may need to stop and query staff before submitting the claim. A reliable **rheumatology charge capture workflow** should make missing details easier to identify before billing.

### 2. Infusion administration details get left out

Infusion coding is not determined by the medication price or by how medically complex treatment feels. The appropriate administration code depends on the service performed and applicable coding rules. Documentation such as the medication, route, start and stop times, and services provided is critical to code selection.

When those details are scattered between nursing notes, the EHR, and charge sheets, coders may spend valuable time tracking down information—or may not have enough documentation to support the intended code.

### 3. Joint injections and ultrasound guidance aren't captured correctly

Rheumatologists perform injections and aspirations across small, intermediate, and major joints. The code depends on the joint and whether qualifying ultrasound guidance was used and documented.

**An important distinction:** ultrasound guidance is included in the ultrasound-guided joint injection or aspiration codes (such as 20604, 20606, and 20611); it is **not** separately billed with those codes. Permanent image recording and reporting are among the requirements for using the ultrasound-guided codes. See the [arthrocentesis and injection coding guidance from ACEP](https://www.acep.org/administration/reimbursement/reimbursement-faqs/Arthrocentesis-Injection-faq).

A missing procedure detail can change which code the documentation supports. Strong charge capture begins with a complete record—not a guess after the patient has left.

### 4. Complex E/M visits can be underdocumented

A new inflammatory arthritis evaluation or difficult follow-up can involve extensive medical decision-making, medication monitoring, comorbidities, and treatment changes. But an encounter should be coded according to the documented **medical decision-making or eligible total time**, not simply how difficult or long the visit felt.

When a significant, separately identifiable evaluation and management (E/M) service occurs on the same day as a procedure, **modifier 25** may be appropriate if the documentation supports it. It is not automatic just because an injection or infusion happened. The [CMS NCCI guidance](https://www.cms.gov/medicare/coding-billing/national-correct-coding-initiative-ncci-edits/medicare-ncci-faq-library) explains the modifier's use.

For revenue cycle teams, the goal is neither overcoding nor conservative guesswork. It is accurate, defensible coding that reflects the work documented.

### 5. Drug wastage isn't documented or reported correctly

For eligible, separately payable Medicare Part B drugs from single-dose containers, reporting requirements may include **modifier JW** for discarded amounts and **modifier JZ** when no amount was discarded. The amount administered, amount discarded, and units billed all need to be reconciled under the applicable rules.

These requirements do not apply to every drug or vial. Your team should verify eligibility, units, and documentation against current payer policy. Review the [CMS discarded-drug guidance](https://www.cms.gov/medicare/payment/part-b-drugs/discarded-drugs) for the current Medicare framework.

### 6. Authorization and charge information fall out of sync

Prior authorizations for specialty medications can change by payer, drug, site of care, and treatment period. An expired or mismatched approval creates a very different problem from a missing charge, but both can disrupt reimbursement.

A charge capture solution does **not** replace the practice's authorization process. It should fit into a workflow where staff can verify that the treatment delivered, documentation, and billing information are consistent—and resolve discrepancies before claims move forward.

**The recurring problem:** valuable work was done, but the record, code, charge, and billing workflow did not stay connected.

## Don't overlook eligible care between appointments

Rheumatology care extends beyond the exam room: medication monitoring, patient education, coordination with other specialists, and follow-up all take time. Some patients may qualify for **chronic care management (CCM)** services, but rheumatoid arthritis alone does not automatically make a patient eligible or make monthly billing appropriate.

Medicare CCM generally requires **two or more qualifying chronic conditions**, along with applicable consent, care-plan, service, time, and billing requirements. Practices exploring CCM should follow the [CMS chronic care management guidance](https://www.cms.gov/medicare/payment/fee-schedules/physician-fee-schedule/chronic-care-management-complex-conditions).

## What should rheumatology charge capture software actually do?

If you're comparing a rheumatology charge capture service, automated medical coding solution, or revenue cycle platform, look beyond the promise of "faster billing." Ask how it handles the actual gaps in your practice.

### Capture the encounter without adding extra steps

Mobile and web-based charge capture can help providers enter or review encounter information closer to the point of care, rather than reconstructing everything after clinic hours.

### Use documented care to support medical coding

AI-assisted coding can review clinical documentation and recommend CPT and ICD-10 codes. For specialized rheumatology workflows, a qualified reviewer should still confirm drug-specific, infusion-administration, modifier, and payer requirements.

### Give coders and billers a clear review workflow

The coding team needs access to supporting documentation, charge status, and a way to address incomplete or questionable items. Visibility can reduce the back-and-forth that slows charge creation.

### Connect EHR documentation to charge capture

An integration between documentation sources, charge capture, and downstream billing systems can reduce unnecessary manual re-entry. Exact data sources and interfaces depend on the practice's existing systems.

### Make missing charges visible

Useful reporting should help teams monitor encounters, missing bills, charge status, time to bill, and coding activity—so revenue leakage becomes something you can investigate rather than an unexplained number at month-end.

## How MediMobile supports a more connected rheumatology revenue workflow

[MediMobile](https://www.medimobile.com/) offers charge capture and AI-assisted medical coding technology for healthcare organizations. The right fit depends on your group's size, documentation sources, coding workflow, and integration requirements.

**Charge Capture:** A centralized web and mobile workflow for entering, reviewing, and managing charges—well suited to practices that want to replace manual or fragmented charge entry.

**Active List:** Builds on Charge Capture with patient visibility, documentation follow-up, reporting, and charge reconciliation tools that help teams identify items needing attention.

## Rheumatology charge capture checklist

Before a rheumatology encounter or infusion moves to billing, ask:

- Is the encounter present, completed, and linked to the correct patient and clinician?
- Is the diagnosis documented at the specificity supported by the record?
- For infusions, are drug identity, dose administered, route, and relevant start/stop times documented?
- Do the HCPCS units agree with the medication record and applicable billing instructions?
- If drug waste applies, are the discarded amount and JW/JZ requirements addressed correctly?
- For a joint injection or aspiration, are the joint, procedure details, and qualifying guidance documented?
- If an E/M service is billed on the procedure date, is the level and any modifier 25 supported?
- Has the coding or billing team reviewed missing information, authorization requirements, and charge status before submission?

A reliable process does not mean every charge automatically gets paid. It means your team has more complete information, fewer avoidable handoffs, and a clearer opportunity to catch issues while they are still fixable.

## Frequently asked questions about rheumatology charge capture

### What is rheumatology charge capture?

Rheumatology charge capture is the process of recording billable rheumatology services and converting documented care—including office visits, procedures, and infusions—into accurate charges for billing review and submission.

### Can AI medical coding help rheumatology practices?

Yes. AI-assisted medical coding can help translate clinical documentation into code recommendations and reduce manual charge entry. Specialized drug billing, infusion services, modifiers, and payer policies still need appropriate configuration and review.

### Is MediMobile a rheumatology medical billing company?

MediMobile primarily provides **charge capture, automated medical coding, charge review, reconciliation, and integration software**. It supports the workflow that gets encounters ready for billing; it should not be confused with a promise that MediMobile performs every part of insurance authorization, claims submission, or collections for a practice.

### What should I ask during a rheumatology charge capture demo?

Bring examples of an infusion visit, a joint injection with ultrasound guidance, and a complex E/M encounter. Ask how the product receives documentation, supports code review, flags missing charges, integrates with your EHR and billing environment, and accommodates your payer-specific requirements.

## Better revenue workflows protect the care patients count on

The patient who can open a jar again will never see your charge queue, your infusion coding edits, or your reconciliation dashboard. But the practice's ability to sustain that care depends on work happening behind the scenes.

The goal isn't to make rheumatologists think more about billing. It's to give providers, coders, and billers a more connected path from documented care to accurate, bill-ready charges.
